Answer 1
6 jumbo bags of ballast + 36 bags of ordinary cement + free delivery because your order will go over £300.

6 x .5cubic.m = 3 cubic.m mixed at ratio of 5 ballast:1 cement needs 12 bags per cubic.m, therefore 3 x 12 = 36 (25kg) bags of cement.

You might have made a mistake on your thickness as the ballast alone will be thicker than that!
Based on a ratio of 1:2:4 (cement:sand:ballast) - Approx values!!

40mm THK - 240KG of cement, 450 KG of sharp sand, 900KG of ballast and 132 litres of water

400mm THK - 2400KG of cement, 4500 KG of sand, 9000KG of ballast & 1320 litres of water

Just in case you did want 4mm THK it would be 24KG:45KG:90KG with 13.2 litres of water

Answer 2
"4mm" thick - do you mean 40mm perhaps, even that is on the thin side. Either way there are some good quantity calculators on the net e.g. try pavingexpert dot com.
